York Health Group

York Health Group is a commissioning consortia made up of 24 GP practices, representing over 230,000 patients, working with NHS North Yorkshire and York to redesign, improve and create new and better local health services for patients.

What we do

• We are commissioning new and better health services out of hospital and closer to people's homes
• We are making it quicker and easier for people to get the health care they need

How we do it

• Working in partnership with the hospital, social services, primary care trust and patient groups, we make decisions about the way patients receive health care
• Being closer to patients, our GP practices are better placed to understand people's health needs
• We look in detail at the health needs of our patients to see if they are being delivered in the best and most cost-effective way
• We redesign and reorganize services so that people can get treatment locally, if that's what they prefer
